Understanding your care needs

From your very first conversation with District Home Care, Woking we will begin building a picture of your individual needs so we can provide the best information and support. We never ‘sell’ our home care services and are happy to discuss your situation and offer advice without any obligation.

If you are ringing to arrange care, we will complete a basic needs assessment on the initial call then agree to visit at a time and location that suits you – this is usually the client’s home but can also be in a hospital or residential care facility, if they are preparing to return home.

Of course we will bring our expertise and answer your questions, but the main aim of this visit is to listen and understand how you would like us to help. We will put this information into a detailed assessment, which, if you decide to accept our services, will form the basis of your Personal Care Plan.

Our care plans are built around the client’s preferred daily routines and the way they like to have things done. We aim to be an unobtrusive source of support, enabling our clients to increase their independence in a safe environment and ensuring complete peace of mind for their loved ones. We will consult health professionals and other members of your support network and take a holistic approach to our clients’ well-being, offering emotional support, companionship and social interaction as well as practical assistance. This approach ensures that our care addresses all your needs and incorporates all your personal choices.
