The care managers ensured we had the right level of support and the necessary equipment before she was discharged from hospital. Prior to my mother coming home, they took the time to visit both our home and the hospital. They met with my mother and spoke to the ward staff, showing a genuine interest in her well-being.

Their kindness, reassurance, and even their sense of humour made a world of difference seeing my mother laugh for the first time since her fall was a true turning point. Thanks to their dedication, my mother felt safe and positive about returning home, despite her initial nerves.
